When choosing implant placement relative to the chest muscle before breast implant surgery, one important factor is the amount or thickness of natural breast tissue.

There is no hard and fast rule regarding breast tissue thickness and ideal implant position, but some surgeons feel that if the breast tissue is less than two centimeters thick, then the implants are better placed below the muscle.

In some circumstances, a patient will decide that she would like the position of the breast implants to be changed after their surgery. Some patients who have the breast implants placed above the muscle would like to have them below the muscle, and some patients who have the breast implants placed below the muscle would like to have the implants above the muscle.
