Gender dysphoria is broadly defined as discomfort or distress that is caused by a discrepancy between a person’s gender identity and that person’s sex assigned at birth. Looking feminine or masculine is extremely important for a trans female or a trans male. Sex reassignment surgeries are far more extensive than the feminizing or masculinizing treatments.
There are standards of care which has specific guidelines provided by the WPATH criteria which is constantly upgraded. The WPATH Standards of Care is issued by the World Professional Association for Transgender Health. This association works world wide for rendering maximum general health, psycological well being and self fulfilmentof persons with gender dysphoria. This includes hormone therapy and or surgical treatments to meet the diverse health care needs of transsexuals, transgenders and gender nonconforming people. These standards of care which are mostly explained from western countries might have differences in the following like social attitudes towards those getting treated like the transsexuals, transgenders and gender nonconforming people, language used to address them, access and treatment cost, legal policies, etc. Whatever treatment provided must be in accordance to these criterias.

Sex Reassignment Surgery for Trans Female [MtF]

Breast/Chest Surgery: Augmentation mammoplasty like breast implants or breast fat transfer.

Hair Restoration: Frontal and temporal recession can be addressed either with scalp advancement or hair transplant.

Reshaping the Nose: Rhinoplasty is always a matter of preference in facial feminization. Fat fill, dermal filler, nose implants or cartilage grafts are the choices, and it could be either an open or a closed rhinoplasty.

Filling the Temporal Fossa: Fat transfer or dermal filler can be used for temporal fossa depression.

Cheek Contouring: Cheek contouring has nothing to do with facial feminization but is done since prominent cheeks gives an aesthetically pleasing appearance. Cheek implants, dermal filler or fat fill are the options.

Lip Reshaping: This includes upper and lower lip reduction, vermillion border reshaping and lip enhancement through dermal filler or fat transfer. Gummy smile correction with Botox is also recommended to give an aesthetic smile.

Chin Correction: Chin plays a highly important role in gender recognition. Increasing or reducing vertical height and width makes aesthetically pleasing appearance. Surgical correction, chin implants, fat transfer or dermal filler can be done to reshape the chin.

Sex Reassignment Surgery for Trans Male [FtM]
Top surgery falls under irreversible intervention which involves breast removal. We follow three types of surgical intervention for breast removal:
Small Breasts:
Liposuction of the chest region with gland excision.
Moderate Sized Breasts:

Stage 1: Liposuction with gland excision

Stage 2: Circum areolar skin excision
Heavy Breasts:
Heavy Breasts (or) People Not Willing for Two Stage Procedure:
Double incision mastectomy with free nipple areolar graft.
Before Surgery
You will be advised to stop smoking for at least four weeks before surgery. Besides, the surgeon shall advice you to avoid certain medications such as Aspirin (blood thinning agents). You will be advised lab tests, the results of which will determine your readiness for the procedure.
Incision
Incision type depends on the amount of excess skin to be removed and its location. An incision can be placed in either one of the below locations:
- Inside of the arm.
- Back of the arm.
- From the underarm to just above the elbows.
After surgery
Following the procedure, dressing is applied along the sutured areas. Small drains tubes are temporarily placed under the skin to drain any excess blood, but not always. You can get back home the very same day or a day later. You will be advised to wear customized compression garments for a couple of months to avoid hematomas and reduce swelling. Sutures, if any will be removed about 2 weeks following the surgery.
The results of a lift are visible almost instantly. But it would definitely take up to 3 or 6 months for the final results to show up. You can resume exercise between 4th and 6th week. Once the swelling subsides and scars settle, you will be immensely pleased with your new look. It confers the additional advantage of improving the dimpled and irregular skin appearance.

You can anticipate 1. Bleeding 2. Infection 3.Poor wound healing 4.Fluid accumulation 5. Numbness 6. Pain 7. Asymmetries 8. Skin Loss 9. Swelling 10.
